Potentiationof Neuromuscular by CalciumChannel Blockade Blockers

A significant reduction was noticed in the amount of vecurenium needed to maintain steady neuromuscular blockade at 20% twitch height (Tl> in patients given nicardipine intraoperatively. Bolus ajection of either nicardipine or d.iltlazem during constant infusion of yecuronium produced transient depression of Tl and the train-offour ratio CTOFR). Reversal of neuromuscular blockade wtth a choline esterase inhibitor (neo-stigrnine} was not retarded by previous administration of Ca-channel blockers but concurrent administration of anticholine-esterase agent and Ca-channel blockers caused a delay in recovery from motor blockade. Monitoring of neuromuscular junctton activity is strongly recommended whenever a large curnulative dose of Ca-channel blockers is used.
